By submitting this form, you are acknowledging that the possibility of my child/dependent/or self being injured while attending Safari Surf School does exist. Surfing has its dangers as does any sport, and I hereby wave, hold harmless, and forever discharge from any liability damages or claims of damages for personal injury, including accidental death, as well as claims of property damage against Safari Surf School, LLC, the US government, the State of California, and their members or agents. I also understand that petty theft does occur in Costa Rica. I release Safari Surf School LLC and Hotel Casa Tucan of any and all liability in event of lost, stolen, damaged or otherwise of personal property. I also acknowledge that by releasing Safari Surf School LLC of any and all liability that it is inclusive of its employees, supervisors, and owners, et al. Safari Surf School LLC is not responsible for lost luggage, delayed flights, missed transportation, etc... I hereby acknowledge that I release Safari Surf School LLC of any liability due to airline service problems.
                                            Safari Surf School, LLC requires a non-refundable deposit of 25% of package total at the time of package booking. The guest's remaining package balance is to be paid no later than 45 days prior to departure date and is non-refundable beyond that time. Safari Surf School, LLC reserves the right to issue a "Rain Check" for guest's deposit and/or package cost on a case-by-case basis if the guest feels he or she may be able to visit Safari Surf School in the near future (within 6 months).
